NYSDOH Issues 2025 Primary Care Core Measure Set Recommendations

The New York State Department of Health (NYSDOH) has issued the 2025 Primary Care Core Measure Set Recommendations.  This updated measure set has been developed through a collaborative effort led by the United Hospital Fund (UHF) and includes input from a diverse group of stakeholders, including consumer advocates, health plans, providers, and other organizations.  The updated measure set aims to align with CMS priorities and initiatives, ensuring that our primary care measures reflect the latest standards and best practices.  The recommended measures span various domains, including prevention, chronic disease, children and adolescents, appropriate use, patient-reported outcomes, behavioral health, health equity, and cost.  Key highlights of the 2025 Primary Care Core Measure Set include:

  • Colorectal Cancer Screening
  • Controlling High Blood Pressure
  • Diabetes HbA1C Poor Control
  • Childhood Immunization Status
  • Screening for Depression and Follow-Up Plan
